The FMGL Regulations, 2021 — at a Glance

Notified by the National Medical Commission (NMC) on 18 November 2021, the FMGL Regulations set the conditions Indian students studying medicine abroad must satisfy to seek registration to practice in India. They remain the governing framework in 2026.

Why These Rules Exist

• Patient safety first — clinical training must be substantial enough to produce a competent, practice-ready doctor.
• Parity with Indian MBBS — the course must be broadly equivalent in duration, theory, and hands-on exposure.
• Closing loopholes — the rules exist specifically to stop degree-mill shortcuts and split/fake internships.

Step 1 — NEET-UG Is Mandatory

If you plan to practice medicine in India after an MBBS abroad, qualifying NEET-UG is mandatory — a foreign degree alone does not make you eligible for registration in India.

• Completed 10+2 with Physics, Chemistry, Biology/Biotechnology and English
• Met the minimum qualifying marks prescribed under applicable regulations

The 10 Rules Every Student Must Know

• Minimum course duration of 54 months (4.5 years), excluding internship.
• Mandatory 12-month internship at the same foreign institution/country — split internships are not accepted.
• English medium throughout; any online component must be compensated with equivalent offline training.
• Curriculum broadly equivalent to the Indian MBBS curriculum, in theory, practical and clinical training.
• Local licensure eligibility — you should be eligible to seek registration in the country where you studied.
• Adequate clinical training and hands-on hospital exposure.
• A recognised medical institution, legally recognised in its own country.
• A further 12-month internship in India is generally required before permanent registration.
• A licensing examination — currently FMGE, until NExT is implemented for foreign graduates (Chapter 06).
• Don't rely on marketing claims — never choose a university based only on low fees or "100% pass" advertising.

For more than two decades, the Philippines has been a preferred destination for Indian medical students. It follows an American-style medical education system, English is the primary medium of instruction, and students get extensive patient interaction through affiliated teaching hospitals.

Advantages & Points to Consider

Russia

Russia has hosted Indian medical students for more than 60 years. Its government universities offer experienced faculty, modern labs, and large teaching hospitals, with English-medium options at many institutions.

Consider: cold climate in many regions; basic Russian is useful for clinical interaction.

Uzbekistan

Uzbekistan has grown popular for its affordability and modern campuses, with a rising number of English-medium programs — but the NMC has specifically advised extra caution when verifying admissions here.

Kyrgyzstan

One of the most affordable destinations, with English-medium programs and a growing Indian student community — verify the specific university carefully.

Living cost: Rs 12,000-20,000/month. Consider: cold winters; basic local language helps during clinical postings.

Vietnam

An emerging destination with modern infrastructure and safe cities. English-medium availability varies by institution — some require Vietnamese for clinical communication.

Living cost: Rs 18,000-30,000/month.

Timor-Leste

A relatively new, emerging option with affordable fees and English-supported instruction at selected institutions. Fewer Indian students are currently enrolled — extra due diligence is essential.

Living cost: Rs 15,000-25,000/month.

Central America and the Caribbean appeal to students seeking English-medium instruction, modern campuses, and (at some institutions) USMLE-focused curricula supporting careers in North America. Tuition and living costs here are generally the highest among the destinations in this handbook.

Living cost: Rs 35,000-60,000/month. St. George's University is internationally known for extensive clinical partnerships but is also among the most expensive options in this handbook.

What Is FMGE?

The Foreign Medical Graduate Examination (FMGE) is a screening exam conducted by the National Board of Examinations in Medical Sciences (NBEMS) for Indian citizens and OCI cardholders who obtain a primary medical qualification outside India and wish to register to practice in India.

FMGE vs. NExT — 2026 Status

The National Exit Test (NExT) is designed to eventually replace both FMGE and NEET-PG as a single licensing exam. As of this edition, FMGE remains the only active licensing pathway for foreign medical graduates, and NExT has no confirmed implementation date for this group — students admitted in 2026 may not face it until around 2031-32 at the earliest. Prepare for FMGE — don't defer preparation in anticipation of NExT.
